Community Health Rep

The Community Health Representative (CHR) is an important part of the One Arrow First Nation Health Centre Team. They help connect community members to health programs, services, and supports. The CHR works to keep the community healthy by sharing information, helping people get care, and supporting different health activities. They work on their own and with a team of health workers to plan and run programs that meet the needs of the community. The CHR is here to support individuals and families and to help everyone stay as healthy as possible.

Community Health Nurse

The Community Health Nursing Program is a special practice of nursing with a unique body of knowledge, requiring a distinct expertise. The Nursing Program supports the delivery of wholistic programs and services within the community, for Elders, families, groups, & individuals. CHNs assess, plan, implement and evaluate health plans to improve and/or maintain the health of the individual/family/community. CHNs will see clients in the clinic and in their home depending on the needs and ability of community members.

Community health nurses (CHN) deliver community-based intervention programs, and these include:

  • Immunization Programs
  • Communicable Disease Control
  • Prenatal and Postnatal
  • Case Management
  • Consultations with Health Professionals
  • Education
Doctor Days

One Arrow First Nation (OAFN) Health works together with the Saskatchewan Health Authority (SHA) to bring doctor services to the OAFN Health Centre three to four days each month. Doctors travel from the Rosthern and Wakaw Medical Clinics to care for community members at the OAFN Health Centre. This service makes it easier and more comfortable for members to see a doctor right in the community. The monthly schedule is shared during the last week of each month and promoted locally. Homecare

The Home Care Nursing Program is a service that provides a holistic view on health that looks at the client’s physical, social, spiritual and emotion needs. Home Care staff will support and help to enhance care provided by family members. Home and Community Care cannot replace the support and care traditionally provided by the family. Home Care is provided in the home, but clients can be seen in the clinic as per their care plan. The Home Care team includes the Home Care Nurse and Home Health Aides.

Home Care Nurses can help with:

  • Nurses Assessment – Reviews health history and helps to make a plan of needs.
  • Nursing care in the home and community
  • Teaching client and family to help with selfcare.
  • Medication Management
  • Supervise personal care.
  • Order equipment and supplies
  • Speak on your behalf and referrals.
  • Teaching how to do own care such as needles, wound care, pills.
  • Help with bathing, foot care, dressing.

Home Health Aides can help with:

  • Personal care
  • Home management
  • Respite services-helping caregiver to care for someone else.
  • Light housekeeping, laundry, meal making.
  • Care while family is away.

Home Care Does Not Provide the following:

  • Replacing care provided by family.
  • Maid Service
  • Taxi
  • 24-hour care
Medical Transportation

One Arrow First Nation (OAFN) Medical Transportation is a program that helps community members get to their medical appointments. This program is provided through a partnership between OAFN Health Services and Indigenous Services Canada. This service is in place for members who do not have a way to travel to medical visits, within the community or outside of it.

Medical Transportation is for medical needs only. After your appointment, you must return a signed Certified Medical Attendance (CMA) form.

Hours:
Regular service hours are Monday to Friday from 8:30 a.m. to 4:30 p.m. To book a ride, please call the Health Clinic at 1 (306) 423-5493 at least 48 hours before your appointment. If your appointment is changed or canceled, it is your responsibility to contact the Health Clinic and let them know as soon as possible.

Evening & Weekend Rides:
If you need a ride in the evening or on weekends, you can call 1 (639) 470-7492 between 5:00 p.m. and 7:00 a.m. If you are leaving the hospital after being admitted, please speak with a hospital social worker to help arrange your ride back to the community.

Maternal Child Health

The Maternal Child Health (MCH) program at the One Arrow First Nation Health Centre supports pregnant women, families with babies, and young children. The program shares helpful information, connects families to services, and helps them learn how to care for themselves and their children. MCH works with other community programs to support healthy living, including prenatal care, Elder support, nursing services, and oral health. Some of these programs include the Canada Prenatal Nutrition Program (CPNP), the Fetal Alcohol Spectrum Disorder (FASD) program, and the Head Start Program.

The MCH program works with the OAFN Health Centre Team to support children’s dental health through the Children’s Oral Health Initiative (COHI). This includes helping manage the program, keeping records, and promoting good dental habits. The goal is to help children build strong, healthy teeth and gums while supporting overall wellness in the community.
